Your first appointment will consist of a diagnostic interview and will be called an intake appointment. After scheduling this appointment you will receive an email requesting that you complete the intake package. This will include the welcome packet, consent forms, and any additional forms that are relevant to the reason for referral. Parents will be asked to complete the child and adolescent intake form for any youth patients. All patients and/or their parents will be asked to complete DSM Level 1 measures along with other screening forms as necessary (Vanderbilt, Adult ADHD rating scale, etc.). All of these forms are web-based and you are asked to complete then at least 24 hours in advance of your appointment so that your clinician is able to review them prior to the appointment. If you choose to use insurance benefits, you will be asked to upload a picture of the front and back of the insurance card along with the patient's date of birth to the intake package so that your clinician can check eligibility and benefits prior to the appointment. If you have a cost share, payment can be made in the form of cash, check or credit/debit (Stripe).
We are located in the Plaza at Belfair, 25 Clark Summit Drive, Suite 103, Bluffton, SC 29910. You may enter the building and the suite is located directly on your right. Your clinician will greet you in the suite at the time of your appointment/conclusion of her previous appointment. At this time we do not have a receptionist to greet you in the suite, nor answer incoming calls to the office. Please be quiet when entering the suite. You may have a seat in the waiting room and check in online. Online check-in is available to alert your clinician you have arrived. If calling the office, please leave a message and you will receive a response within 1-2 business days. While you are waiting for your appointment, please note that other professionals may also be working in the suite so quiet conversations are appreciated to reduce disruption to other ongoing sessions.
If you are coming for psychological testing services, your clinician will likely need to request prior authorization from the insurance company before proceeding. This will occur after the intake appointment. Once authorization is obtained, testing can take place which will occur over 1-2 sessions (depending on needs for the evaluation). After testing is completed and all forms/questionnaires from patient/observer/teacher, etc. are returned then the feedback session will be held. You will receive a psychological report at the feedback session. **Please note, insurance does not cover educational testing and therefore, any requests/needs for academic testing will be private pay only.
